Your local Alliance manager has arranged for SwiftDocs & Fingerprints to handle FD-258 fingerprint card processing required for your out-of-state work assignments. Please review the details below before booking your appointment.

• Service: FD-258 Fingerprint Card (Ink or Livescan Printed to Card)
   
• Purpose: Required for out-of-state work assignments
   
• Fee: $25 flat rate — all taxes included
   
• Turnaround: FD-258 card provided immediately at the end of your appointment
   
• Location: SwiftDocs & Fingerprints – Kissimmee, FL

 Please bring the following to your appointment:

• One (1) valid, government-issued photo ID
  (Examples: Driver License, State ID, Passport, Military ID)
   

If your assignment requires more than one FD-258 card, additional cards may be requested during your visit at cost of $10 per additional card.
